Before you apply please refer to our subject access request guidance notes

To apply for a subject access request, tell us:

  • Who the data subject is: name, address, contact (you can use our form to request information on up to two people)
  • Agent details (if applying on behalf of someone else)
  • The information you require

Every application must be witnessed and accompanied with identification for each data subject (and agent, where applicable).
