Hinckley and Bosworth Health and Wellbeing Partnership
Terms of reference
Purpose
To improve both health and wellbeing and to reduce inequalities for local people, through partnerships of key local organisations.
Aims
- To improve the health and wellbeing of people in the Hinckley and Bosworth borough, and to reduce the inequalities in health experienced by some social groups or people living in geographical areas of greater social need
- To identify and agree priority outcomes for health and wellbeing in Hinckley and Bosworth
- To complement the Integrated Neighbourhood Team (INT) monthly meetings that are held locally
Objectives
- To align workstreams captured within the Joint Leicestershire Health and Wellbeing Strategy
- To focus on outcomes and demonstrate improvement in health and wellbeing
- To monitor and provide scrutiny of the key agreed actions contained within the Hinckley and Bosworth Community Health and Wellbeing plan by supporting other local plans
- To ensure themes such as prevention and collaborate working underpin delivery
- To provide strategic local direction regards to the Hinckley & Bosworth Community Health & Wellbeing plan
- To ensure community involvement and engagement in the development of health and wellbeing plans, using opportunities to link up relevant community involvement activities where possible
- To support effective joint commissioning and monitoring of joint work to improve health and reduce health and social inequalities
- To harness new opportunities with the voluntary sector to promote health and wellbeing and tackle health and social inequalities
- To ensure that resources are targeted appropriately, to ensure maximum efficiencies are based on best evidence
